Order supplies by fax

Supplies required for specimen collection and transport are provided to our clients free of charge. Please call or fax your order to the number on the form below. Forms can also be given to our courier.

Supplies must only be requested and used for testing referred to Pathology Laboratories. Pathology Laboratories and the health care provider are liable under the Federal Anti- Kickback Statute for violations of this statute.

Note: This Supply Order Card is only for use by clients of PathLabs, Midwest Division of SHUSA, in Toledo, Ohio.

It is NOT for use by clients of CPL (Austin, TX).
